Read The Following Instructions Carefully

There are two parts in the Certification application process.

Part 1

1. Click the button below to download the application form
2. Take a print
3. Fill out the printed form with a black pen
4. Scan the filled form 
5. Send the scanned copy of the filled form to salivasummitindia@gmail.com

Part 2:

Inside the form, there is a “Click Here” option at the end of the pdf. Click on it and follow the instructions to upload your digital signature online.
